This section is designed to capture information about the patient’s existing long-term medical conditions such as diabetes, hypertension, asthma, or any other chronic illness. It helps the doctor understand the patient’s medical background and current health status.

  • The diagnosis autosuggest field allows patients to quickly select their known conditions from a predefined list, ensuring accuracy and standardization.
  • The follow-up question about new or worsening symptoms helps identify any recent changes or complications related to these conditions.


Step 1 : Go to Analytics


**Step 2: **Click on Questionnaires.


  • View the Self-Assessment data
  • Go to the top-right corner (right-hand side)
  • Click on the menu option
  • Select Download to download the data
