How to Create a Consent Form?


Doctors can create and share consent forms for patients directly from the platform. You can either create a new consent form or use an existing template.



Step 1: Once the patient is checked-in, they will appear under My OPD


  • Under the specific patient, click on more option (** menu )**




Step 2: Select “Create Medical Certificate”






Step 3: Under Consent Form, choose:
  • Make your own (to create a new form), or Select a pre-existing template






Step 4: Add the required details
  • Click on Continue





Step 5: Download, print, or share the consent form with the patient





Important Notes


  • Ensure the patient is checked-in to access this option
  • You can reuse existing consent form templates for faster workflow
  • Forms can be downloaded, printed, or shared digitally
