Overview
The Appointment Booking Form configuration allows clinics to customize the information collected from patients when they book an appointment. This helps ensure that the clinic gathers all the necessary details in advance for better patient management and smoother appointment scheduling.
Step 1: Click on Queue from the left sidebar.

Step 2: Click on + New to book a new appointment.

Step 3: To enable additional fields, click on Advanced Settings.

Step 4: A pop-up will appear displaying the available fields that can be enabled.

Step 5: You can change the order of the additional fields using the three vertical lines on the left side of each field.

Step 6: To add a field, check the checkbox. To remove a field, uncheck the checkbox.

Step 7: After selecting the required fields, click on Save.

Step 8: Once saved successfully, the selected additional fields will appear in the appointment booking form.
